What does 'per sempre' mean? I know it's Italian, but that's about it.

"Ultras per sempre" means Ultras for always or for ever I think.

Yeah, literally it's "Ultras for always" but it should be translated as "Ultras forever".

The word "sempre" is used in yet another way in a soccer context. It is common for Italian commentators giving the score during a match to say, for example, "sempre uno-uno", which literally means "always one-one" but which should be translated as "still one-one".
